STUDENT ORGANIZATION CAMERA REQUEST FORM

- Camera is available for use by all REGISTERED student organizations.
 
- Camera was purchased by USG in December 2007. 
 
- Camera and all equipment are to be held by SLS and treated as a regular piece of SLS inventory.  Any repairs or maintenance that are not covered by warranty or insurance shall be covered by the USG Reserve Fund.
 
- Factors in deciding who shall rent the camera will include, but are not limited to: Necessity of camera to organization's function, Importance of event to campus.
 
- MAXIMUM PERIOD of checkout is 2 WEEKS at a time.
 
- Organizations must return the camera to SLS over any break from school, ie. Winter, Thanksgiving, Summer.
 
- Camera must never leave Houghton or Keweenaw County.
 
- LATE RETURNS shall incur a $10 fee per day to their USG account. All items, cables, and accessories must be returned.
 
- A DEPOSIT of $20 will be collected upon camera checkout. This will be returned upon complete return of camera.
